These challenges can make losing weight – and keeping it off – far more difficult than popular advice often suggests.


NHS Tier 3 weight management services are designed for

  • people living with severe or complex obesity

Rather than focusing on diet alone, Tier 3 services bring together experts from a range of healthcare professions to provide personalised, long-term support.

What does Tier 3 mean?

The NHS uses different tiers to describe levels of weight management support.

In general terms:

  • Tier 1 focuses on promoting healthy lifestyles across the whole population through public health initiatives.
  • Tier 2 provides structured community-based weight management programmes centred on nutrition, physical activity and behaviour change.
  • Tier 3 offers specialist, multidisciplinary care for people living with severe or complex obesity.
  • Tier 4 includes weight loss surgery services for people who meet strict eligibility criteria.

Sitting between community weight management programmes and bariatric surgery, Tier 3 provides more intensive medical support for people whose needs go beyond standard lifestyle advice.

Who are Tier 3 services designed for?

Eligibility for and delivery of Tier 3 services varies across the country, depending on NHS availability.


Generally speaking, these services are intended for adults living with severe or complex obesity who would benefit from more specialist support. This specialist support goes beyond what primary care (such as GP surgeries) can provide.


When considering a referral to a Tier 3 weight management service, healthcare professionals look at much more than a person's weight. They may consider factors such as:


  • Body mass index (BMI)
  • Obesity-related health conditions
  • Previous weight loss attempts
  • Overall health
  • Whether specialist intervention is likely to improve long-term outcomes

If you're unsure whether Tier 3 support is appropriate for you, ask your GP or healthcare professional. They can advise based on your individual circumstances and local referral criteria.

What support do Tier 3 services provide?

One of the main differences between Tier 3 services and standard weight loss programmes is the breadth of expertise available to patients.


Those referred to Tier 3 services are often supported by a multidisciplinary team that may include specialist doctors, dietitians, psychologists and exercise professionals. This reflects the growing understanding that obesity is a complex, long-term health condition influenced by biological, behavioural, psychological and environmental factors.


That means support extends far beyond calorie counting or meal plans. The focus is on improving overall health, identifying weight loss barriers and helping people develop sustainable habits they can maintain long after the programme ends.


Why additional support matters

Regardless of your motivation, life happens. Stress, anxiety, poor sleep, emotional eating, previous dieting experiences and mental health all influence eating behaviour and can make weight management significantly more challenging.


That's why psychological and behavioural support is often a central part of Tier 3 care. Patients may work with specialists to better understand unhealthy patterns or habits, establish realistic goals, build confidence and develop practical strategies for managing setbacks.


By addressing the underlying behaviours that affect weight, this support can help create lasting changes that continue beyond the programme itself.


Do Tier 3 services prescribe weight loss medication?

Depending on your clinical circumstances and local commissioning arrangements, a Tier 3 service may recommend prescription weight loss medication as part of your treatment plan.


This could include medications such as Wegovy or Mounjaro where clinically appropriate and available.

These medications are clinically proven to be safe and effective when prescribed by a healthcare professional. In clinical trials:

  • Tirzepatide (Mounjaro) users lost an average of around 21% of their starting body weight over 72 weeks.¹
  • Semaglutide (Wegovy) users lost an average of around 15% over 68 weeks.²

But medication isn't suitable for everyone, and it's rarely used on its own. Instead, it forms part of a wider programme that also includes diet changes, physical activity and behavioural support.

What happens once you're referred?

Most Tier 3 services begin with a comprehensive assessment designed to build a detailed picture of your health and circumstances.

This typically includes a review of your:

  • Medical history
  • Current medications
  • Weight history
  • Eating habits
  • Activity levels
  • Mental well-being
  • Personal goals

Clinicians then use this information to develop a personalised treatment plan reflecting your specific needs and challenges.


As treatment progresses, regular reviews help ensure that support remains relevant and effective. Your care plan can be adjusted over time as your health improves, your goals change or new challenges arise.


Our role at Semapen

Semapen is one of the providers delivering NHS-funded Tier 3 weight management services through the Any Qualified Provider (AQP) framework.


Eligible patients referred through the NHS can access support remotely using the Semapen app. This reduces the need for frequent in-person appointments and helps make specialist care more accessible.


Where clinically appropriate, treatment may include GLP-1 medications such as Wegovy or Mounjaro. For others, support may focus on non-medication approaches, including structured total diet replacement (TDR) plans.

What if you don't qualify?

While not everyone will meet the referral criteria for NHS Tier 3 services, it doesn't mean support isn't available.


Many people choose to access weight management programmes privately, particularly if they don't meet local NHS eligibility criteria or would prefer to begin treatment sooner.
